Operations in Code systems
DELETE /api/v1/code-systems/{codeSystemId}/concepts/{conceptId}

A code still on a patient's record, such as a visit's procedures, or on a saved price is refused with 422.

The integration client needs CODE_SYSTEMS:UPDATE. A practice owner grants it under Permissions on the client.
